Why your body feels on edge
Years of heavy drinking force your brain to lower its internal brakes and crank up its accelerator system. When you quit, this accelerator remains stuck on high alert, causing a surge of stress hormones, physical anxiety and exhaustion.
Practical steps for relief
You can actively protect your nervous system by reducing environmental stimulation. Focus on cutting out stimulants like caffeine, shifting to low-intensity movement like walking, and using breathing techniques to quiet panic, and nourishing your body with good nutrition.

After years of trying, I'd finally stopped drinking. Utterly sick and tired of feeling sick and tired, I certainly expected to feel healthier and a little more at peace. I thought clearing the hurdle of physical detox meant the hardest part was behind me.
So when that didn’t happen, I remember looking at my life and thinking, What the hell? Why do I still feel so sick and tired?
I was blindsided by sudden waves of anxiety, severe brain fog, erratic mood swings and a fatigue I just couldn't shake. My inner critic immediately took the wheel, telling me that I was failing at recovery, that my sobriety was somehow broken, or that I was simply losing my mind.
What I was experiencing back then is a well-documented neurological process called post-acute withdrawal syndrome, or PAWS.
While acute withdrawal is the body's immediate response to removing alcohol, lasting a week or two, PAWS is the much slower, behind-the-scenes process of the central nervous system trying to recalibrate. Put simple, your brain is rewiring itself after years of chemical disruption and that process is rarely comfortable.
Post-acute withdrawal syndrome: The science
To understand PAWS, we have to look at your brain's ultimate goal: balance, or homeostasis.
When you drink heavily over a long period, your brain relies on alcohol to calm it down. To stop you from passing out, your nervous system fights back by turning down its natural calming chemical (GABA) and cranking up its main energising chemical (glutamate) to keep you awake.
When you stop drinking, the alcohol disappears, but your brain is left stuck on high alert. You are left with a massive overload of that energising chemical and almost no calming chemicals to balance it out. This neurochemical mismatch puts your system into a temporary state of electrical overdrive, which is exactly what triggers physical panic, muscle twitches, and insomnia.
At the same time, chronic drinking breaks your body's internal alarm system, leaving it continuously pumping out stress hormones like cortisol. This keeps you trapped in a physical fight or flight response, with a racing heart and shallow breathing, even when you are just sitting safely on the couch.
Finally, withdrawal disrupts how your body handles tryptophan, the basic ingredient needed to make your joy and sleep chemicals (serotonin and melatonin). With your supply lines cut, your brain is essentially running on an empty tank, directly causing the fragmented sleep, vivid dreams, and low mood that many experience months into recovery.
How it appears: The roller coaster of symptoms
Your brain's internal rewiring does not happen in a neat, straight line. Instead, PAWS moves in waves and windows.
You might enjoy a beautiful window of a few weeks where you feel clear, calm and like your old self, only to wake up the next day in a dark wave of irritability, panic, or a flat, numb feeling where nothing brings you joy. This cyclical nature is incredibly frustrating, but it is the classic hallmark of a healing nervous system.
Here's how PAWS can show up:
Cognitive exhaustion: Feeling brain fog and struggling to focus on complex tasks.
Emotional instability: Panic or anger over tiny inconveniences, or anhedonia, which is the temporary inability to feel pleasure or joy.
Physical and neurological sensations: Muscle twitches, tingling or burning sensations in the hands and feet and dizziness.
Sleep problems: Falling asleep but waking up constantly, experiencing disturbing dreams.
Many people mistake a PAWS wave for a sign that they cannot handle sobriety, using it as a justification to pick up a drink. But these symptoms may just be your brain repairing itself.
An action plan for your nervous system

The good news is that PAWS is usually temporary. Your brain knows exactly how to return to balance; it just needs time and a stable environment to do the work.
While your nervous system is busy rewiring, here is how you can support your body and make the ride smoother:
1. Guard against extra stimulation
Because your brain is already operating with too many accelerator pedals active, you want to avoid pouring fuel on the internal fire. High doses of caffeine, for example, act as direct stimulants on an already hyper-reactive nervous system, potentially triggering a PAWS panic wave. Consider switching to decaf or herbal teas.
2. Pace your physical movement
While exercise is fantastic for mental health, exhausting, high-intensity workouts can spike your cortisol levels and trigger an energy crash. Swap gym sessions for gentle walks or yoga until your system stabilises.
3. Quiet the panic response
When a wave of physical anxiety hits, you cannot always reason your way out of it, but you can change your physiology. Use the physiological sigh: take two quick inhales through your nose (one deep breath, followed by a sharp extra sip of air at the very top), then let out one long, slow, vocal sigh through your mouth. Doing this three to five times mechanically forces your vagus nerve to slow your heart rate down, signalling to your brain that you are safe.
4. Support your biochemistry with nutrition
Eat regular meals of whole, fresh foods and drink plenty of water.
5. Re-frame the waves with a journal
When a wave hits, your mind will tell you that you are back at square one. Do not believe it. Think of a wave like the loud, disruptive construction noise outside your house when a road is being repaired. It is annoying, but it means essential work is being done. Keep a simple daily symptom journal. Over time, looking back at the pages will prove to you that the windows of wellness are getting wider, and the waves are becoming shorter and less intense.
Be patient with yourself. Recovery is a long-term project and giving your brain the time it needs to heal is the ultimate act of self care. How long does PAWS usually last?
Generally, these symptoms start creeping in a few weeks into sobriety and can fluctuate anywhere from a few months up to a couple of years. The most important thing to remember is that it does not last forever; your brain is highly resilient and steadily returns to its natural balance over time.
How can I tell the difference between PAWS and a psychiatric problem?

Can a brief slip-up or small drink reset my PAWS timeline?
Unfortunately, it can. This happens due to a process called the kindling effect, where your central nervous system retains a physical memory of every past withdrawal attempt. Because the brain becomes hyper-sensitised, even a single drink can cause your internal accelerator pedal to jam right back down, resetting your symptoms or making the next wave feel even more intense.
